For a form based authentication, enter the username in the Username field and password in the Password field. 

If you want the system to remember your login, check the Remember me for 7 days.

Click on the Login button.

Forgot Password

If you have forgotten your password, click on the Forget Password link.

Enter the username in the Username field. Click on Submit.

Check your mailbox for an email with the new password. You will be required to change the password after you have logged in.
